Community Health Assement

Central Valley Health District and Jamestown Regional Medical Center are working together to update the Community Health Assessment (CHA) and Comminuity Health Improvement Plan (CHIP) for Stutsman County. More information about the CHA/CHIP can be found at the Community Health Partnership webpage.

The survey will take approximately 10 to 15 minutes to complete and can be done online or on paper. A paper copy can be picked up at the reception/admission desk of Central Valley Health District, Jamestown Regional Medical Center, Sanford Health, and Essentia Clinic. The paper survey includes a self-addressed stamped envelope for returing the survey.

The deadline to complete the survey is March 15, 2018.

Diabetes Prevention Program Starting January 2018

Take control of your health and start making positive lifestyle changes with the help of a trained lifestyle coach and fun, engaging support groups. The sessions start January 10, 2018 and run from 12:00 - 1:00PM.
