Who Qualifies?
- Primarily age 50 & older
- Willing to participate fully
- Live in Stutsman, Logan, or Barnes County
- Those with or without health insurance
- Chronic Medical Condition (listed below)
- Heart Failure
- Diabetes Type II

- Chronic Obstructive Pulmonary Disease (COPD)
- Hypertension
- Coronary Artery Disease (CAD)
- Alzheimer's/Dementia

- Nursing Visits
- Assessments
- Goal setting
- Prevention and Education
- Refer as appropriate
- Medication Set Up
- Assure up to date vaccination status
- Personal Health Journal

- Coordination with Primary & Specialty Providers
- Services provided in client’s home or in the local public health office
